Step 1: Contact us now to complete a brief screening survey.

 

This short screening survey is completed by a member of our Assessment Team on the phone. We will ask you general questions about your child's anxiety and will take about 10-15 minutes of your time. This will let us know what type of care your child might qualify for.

Step 2: Once completed, our team will contact you to notify you of your status and whether you have been advanced to Part 2 of the screening process. 

 

 

Step 3: After you are cleared for advance by our team, you will begin Part 2 of the screening process.

 

Part 2 includes both you and your child coming in to our clinic for a more in depth interview about your child's anxiety. The Part 2 Interview will take approximately 1.5-3 hours. Ycan download the required intake information sheet for this appointment here.

 

Please note that if your child does not meet requirements for the study, our team will glady provide you with local, trusted referrals for professionals that we believe best meet you and your child's needs.
